Participation
Students who wish to participate in a high school athletic program must obtain the consent of their parents or guardians.
Parents or guardians who consent must:
- Review the documents below.
- Fill out the "SSFUSD Athletic Packet".
- Ask a medical professional to sign, date, and mark the appropriate box on page 2 of the "Athletic Pre-Participation Screening Exam" form.
- Submit the completed "SSFUSD Athletic Packet".
An online tool provided by SportsNet Inc. is available to help parents submit the required forms.
Insurance for students is available through Pacific Educators Insurance Services at 1-800-722-3365.
Stanford Medicine's Arbor Free Clinic also provides acute clinical care for adults who lack health insurance.
